Origin of the Medicine
Origin of the Medicine
Synopsis & Key Lore
- Oleg’s Message & Clinic Overload: Oleg messages the Trailblazer using a mobile device provided by March 7th. Deeply concerned that Natasha has been working without sleep as the sole medical pillar of the Underworld, he asks the Trailblazer to check in on her.
- The Mutual Aid Society Suspicion: At the clinic, Natasha reveals that multiple patients with severe ailments have abruptly stopped coming for treatments. She suspects the so-called “Complex Illness Mutual Aid Society” is peddling pseudo-scientific remedies and asks the Trailblazer to investigate undercover.
- Interrogating the Missing Patients:
- Depressed Patient: Refuses treatment out of pride and hypochondria, claiming he cannot afford running up a debt at the clinic. He brings up Dr. Vache—Natasha’s brother who previously provided free medicine in Rivet Town—and whispers rumors that Natasha caused Vache’s death out of jealousy. The Trailblazer persuades him to return to the clinic.
- Elderly Patient: Hard of hearing and grieving her late husband, who sacrificed his own medicine for her before dying in the mines. She feels like a burden, but is touched by the Trailblazer’s encouragement and agrees to see Natasha.
- Suspicious Patient: Bitterly denounces Natasha’s slow, methodical treatments as useless idealism for poor miners. He reveals that Dr. Vache’s “wonder medicine” offered instant pain relief despite known lethal risks, and shares the secret trade location of the Mutual Aid Society in Rivet Town.
- Perspectives in the Clinic: Speaking with clinic patients, the Trailblazer hears conflicting accounts: some view Vache as a saintly “people’s doctor,” while others recognize that he had zero medical ethics and treated vulnerable miners as disposable lab rats.
- The Tragic Victim: Natasha directs the Trailblazer to witness the real-world consequence of the wonder medicine: the Lonely Woman, who suffered a total mental collapse after her husband and child rapidly deteriorated and died from consuming Vache’s experimental concoctions to cure a mild ailment.
- The Dark Truth of Dr. Vache & “Blizzard Immunity”:
- Natasha confesses that she and Vache were not biological siblings; she was an orphan adopted into a prestigious Overworld medical family.
- Vache became fanatically consumed by a single project: Blizzard Immunity, seeking to genetically condition human bodies to survive the Eternal Freeze so civilization could reclaim the Snow Plains.
- Lacking human subjects in the Overworld, Vache came to the Underworld and deliberately simulated mild Fragmentum corrosion on unknowing patients, masking their suffering with pain-killing reagents while observing biological changes.
- When his unethical human experimentation was uncovered, Oleg and Natasha permanently banished him beyond the city gates into the blizzard of the Snow Plains to prevent an angry mob from killing him.
- Dismantling the Counterfeit Ring: The Trailblazer tracks down the Mutual Aid Society in the ruins of Rivet Town, defeating their guards and capturing the Society President, who confesses that they were merely exploiting desperate patients for profit. Natasha arrives, secures all remaining reagents for incineration, and vows to never abandon her patients.
Dialogue & Mission Progression
1. Receive a Message from Oleg
(The Trailblazer receives an incoming message on their mobile device)
- Oleg: {NICKNAME}, can you receive this message? March 7th gave me this little thing and taught me how to use it. Well, it seems you can teach an old dog new tricks after all. It just takes time. I kinda know how it works by now. Nice to have something that lets me talk to people without meeting them face to face. Haha.
- [Trailblazer] You learn quick.
- Oleg: I’m flattered, {NICKNAME}. Now let’s cut to the chase.
- [Trailblazer] What is it, chief?
- Oleg: Hahaha, even you have started calling me that? Anyway, let’s cut to the chase.
- Oleg: I heard Natasha is seeing a lot of patients and hasn’t slept for days. She’s the backbone of the Underworld, and we can’t afford to have her burned out. {NICKNAME}, I know you did a great favor for her recently, but can I ask you to check on her again just to make sure she’s alright?
- [Trailblazer] No problem. I’ll go right away.
- Oleg: Haha, I knew I could count on you the first time we met. I was not wrong after all. Thank you so much.
- [Trailblazer] She’s strong. She should be fine.
- Oleg: Haha, I agree. She can be an overachiever. In any case, a little extra help is always welcome. Thank you in advance.
2. Talk to Natasha in Boulder Town Clinic
Step Description: You receive a message from Oleg, who hopes you can visit Natasha and see what she’s up to. Every day, she works tirelessly to look after the patients, so Oleg hopes you can offer her some help.
(Inside Natasha’s Clinic in Boulder Town)
- [Trailblazer] Need any help, Dr. Natasha?
- Natasha: You came at a good time, {NICKNAME}, there is something you can help me with.
- Natasha: According to my patient records, there are a few that haven’t come for their follow-up visits. It happens from time to time, but never as regularly as this. I’m a bit concerned… Have you heard of the Complex Illness Mutual Aid Society?
- [Trailblazer] Sounds like a charitable organization.
- [Trailblazer] Sounds suspicious…
- Natasha: Mm, I agree. Generally speaking, mutual aid societies like these allow patients to share their experiences and support each other. It’s a way of countering all the negative emotional effects of being ill… But there might be more to this society than that.
- Natasha: I’ve got a few patients who’ve started refusing treatment since joining the society… It certainly makes you wonder.
- Natasha: The most suspicious part is that no matter how much I ask, no one’s ever willing to tell me anything about it… and that’s where you might be able to help.
- [Trailblazer] Got it, you want me to go undercover.
- [Trailblazer] Do I need to be ill before I can make inquiries?
- [Trailblazer] What should I do?
- Natasha: Pretend to be a patient. Help me investigate this so-called “Mutual Aid Society” and find out exactly what they’re up to.
- Natasha: If I find out they’re preventing me from doing my job… or if they’re bringing harm to my patients… I won’t just sit on the sidelines. Can I entrust you with this task, {NICKNAME}?
- [Trailblazer] No problem, leave it with me.
- [Trailblazer] Have I ever refused you?
- Natasha: You’re as considerate as always, {NICKNAME}. Good luck. I’ll be sure to thank you properly when you’re back.
3. Track Down the Missing Patients
Step Description: As the chief of Wildfire and the only doctor in Boulder Town, Natasha is busy every day. Do her a favor and persuade the patients who joined the so-called “mutual aid society.”
Patient 1: The Depressed Patient
(In the streets of Boulder Town near the Goethe Grand Hotel)
- Depressed Patient: You… I’ve seen you at the clinic. You run errands for Natasha, right?
- [Trailblazer] Well, there goes my cover…
- [Trailblazer] Uh… Who’s Natasha?
- Depressed Patient: sigh Let me guess, Natasha wants you to persuade me to go back for a follow-up visit, right? What’s the use? I’m terminal. Life’ll be over before I know it.
- [Trailblazer] Was that Natasha’s diagnosis?
- Depressed Patient: Of course not, I read it in a book. Every book I read has descriptions that match my symptoms exactly… I can diagnose myself with a dozen incurable diseases before I get halfway through. For people in my situation, a follow-up visit is just a waste of time and money… not to mention medicine.
- [Trailblazer] Soooo nothing to do with you joining the mutual aid society then?
- Depressed Patient: Mutual aid what now? What are you talking about? Natasha was seeing me free of charge. She said I could keep a tab running for as long as I liked, then pay it back when I had the opportunity… I guess I’m too proud.
- Depressed Patient: sigh …Dr. Vache never charged me anything either. Natasha’s great, but Vache was always my number one…
- [Trailblazer] Who’s Dr. Vache?
- Depressed Patient: Natasha’s brother? They both used to practice medicine back in Rivet Town. I heard he was a top student from the Overworld medical academy — Belobog’s main hospital wanted him. I’ve no idea what convinced him to set up down here… a kind heart, maybe.
- Depressed Patient: Dr. Vache was a people’s doctor — he handed out free medicine. He even followed up with detailed questions on how we felt. The Underworld is always short on medical resources — they say that Dr. Vache made all his own medicine himself.
- [Trailblazer] Why don’t you go to him anymore?
- Depressed Patient: You think I don’t want to? I heard that he… passed away. Keep your voice down… Don’t go telling anyone else about this next bit: I heard that Natasha had something to do with Dr. Vache’s death. They say she was jealous that her brother had stolen her patients, so she…
- [Trailblazer] And you believe that?
- Depressed Patient: Agh, it’s just something people were saying… I don’t think Dr. Natasha could do something like that. Forget I said anything. Since Dr. Natasha sent you over specially, I’ll do her a favor. I’ll go to the clinic for my follow-up a little later.
Patient 2: The Elderly Patient
(Near the food stalls in Boulder Town)
- Elderly Patient: …What was that? You’ll have to be louder! My ears aren’t what they used to be — everything’s muffled.
- [Trailblazer] I wanted to ask why you’re not going for your follow-up visit.
- Elderly Patient: Hah? Follow-up visit? sigh Natasha’s a wonderful girl. She always gives me a thorough checkup… But at this age, it’s rather a lot to bear. Even if they could cure me, I’m sure I wouldn’t be around for long. I don’t want to waste anyone’s time…
- Elderly Patient: That girl’s always trying to get me to take my medicine on time. My husband had the same illness as me, but he never touched the medicine. He wanted to make sure I had enough. Then one day, he said he needed to take care of some things at the mine. He never came back…
- Elderly Patient: I miss him. Now that he’s gone, who cares how long I stick around?
- [Trailblazer] Dr. Natasha cares.
- [Trailblazer] I care.
- Elderly Patient: You don’t give up easily, do you? Still… you’re full of kind words. Okay, you win. It’s off for another follow-up visit with Dr. Natasha, I suppose…
- [Trailblazer] Have you heard of the mutual aid society?
- Elderly Patient: Virtual who? …No, I’ve never heard of any mutual aid society…
Patient 3: The Suspicious Patient
(In an alleyway near the Boulder Town Fight Club)
- Suspicious Patient: Why haven’t I gone for my follow-up visit… Are you a patient too, or did Dr. Natasha send you over? Tell her to save her worries and medicine for other people.
- [Trailblazer] Did you have a dispute with her?
- Suspicious Patient: A dispute? No, no, she’s the only kindhearted doctor in the Underworld. I just don’t really approve of her methods, that’s all. Too idealistic, and no real efficacy… She tries to save everyone, which means she can’t save anyone… In the end, it’s the patients who suffer, and she just tires herself out.
- Suspicious Patient: Miners like me don’t have time for lengthy treatments… If I can’t take something and get right back to work, I may as well lie down and wait for death to come knocking…
- Suspicious Patient: Did you know Dr. Vache? His wonder medicine is cheap and the results are immediate. You just need to be willing to… take a little risk.
- [Trailblazer] “Risk”?
- Suspicious Patient: Dr. Vache’s medicine is… different. Lots of people make a full recovery after taking it… but some get worse, and the unlucky ones check out early. When life forces you to the edge, you’ll throw yourself at any opportunity… even if it means risking it all. For people like us, a slow and tortuous treatment is the same as a death sentence. If it’s between that and a quick result, we’ll take the latter.
- [Trailblazer] Where can I go to get this wonder medicine?
- Suspicious Patient: The Complex Illness Mutual Aid Society is still selling that medicine… I can tell you where they trade in Rivet Town, but there’s a rule: you must not tell any of this to Dr. Natasha, you hear? Here, take this, it’s the address.
4. Debrief with Natasha & Inquiring at the Clinic
(Return to Natasha’s Clinic in Boulder Town)
- [Trailblazer] Explain what you found out about the mutual aid society.
- Natasha: …So the mutual aid society found my bro— …Vache’s research results… and now they’re selling this so-called “wonder medicine” to patients…? Any organization doing something like that is interested in more than just “mutual aid…”
- [Trailblazer] The patient said he was willing to take the risk.
- Natasha: …As a doctor, I don’t accept that — I’m responsible for their well-being. My attitude towards this “wonder medicine” won’t change. What Vache left behind is nothing more than reagents and experimental by-products… The word “medicine” doesn’t apply here.
- Natasha: If you’re curious about Vache’s wonder medicine, you should ask some of the patients in the clinic… see what they have to say about the effects.
(Speaking with patients inside the clinic ward)
- Emotional Patient: The medicine Vache left behind? That stuff… Can you even call it medicine? Vache had zero medical ethics — he didn’t deserve the title “doctor.” Whenever I went to see him, he never even asked about my illness! He just prescribed me his so-called “wonder medicine…”
- Calm Patient: Some people say that Dr. Vache was a saint who came from the Overworld… I don’t buy it. Vache couldn’t have cared less about the people taking the medicine… The hard truth is that we were just his test subjects. It didn’t matter if the medicine worked or not. What mattered was having a constant stream of willing lab rats…
5. The Tragic Victim: The Lonely Woman
(Speak with Natasha, then proceed to Boulder Town plaza)
- Natasha: {NICKNAME}, I want you to go and visit another patient — a victim of the wonder medicine… I want you to see the real consequences of abusing that medicine. Once you’ve understood her situation, come back and tell me what you think.
(Approach the Lonely Woman sitting motionless on a bench)
- Lonely Woman: …Who are you…? Where’s my husband… and where’s my child…? Wh—Where are they? You… you know where they are, right?
- [Trailblazer] Try and stay calm…
- Lonely Woman: …You don’t know… they don’t know… nobody knows… Where… Wh—Where did they go…
- Kind Old Lady: Come over here, child. It’s best to leave that poor lady in peace. Both her husband and child became ill and passed away. Before her family passed away, she was a carefree young woman.
- Kind Old Lady: Her husband and child caught a mysterious illness. The symptoms weren’t grave, but their livelihood was certainly impacted. In order to try and rid themselves of the ailment once and for all, they bought some kind of “wonder medicine…” Who would’ve thought their condition would deteriorate so rapidly after they began using it.
- Kind Old Lady: The two began to suffer terribly, such that death became a blessing… It wasn’t long before they departed this world, one after the other. From that moment on, oh she became as you see her now. If Dr. Natasha hadn’t taken her under her wing… well, it doesn’t bear thinking about.
- Restless Patient: That Dr. Vache wasn’t human… he was a devil in a doctor’s coat! At least the mutual aid society doesn’t hide the risk from the buyers. Vache gave that poison away to everyone for free… even the elderly and the children…
6. Natasha’s Confession: The Dark Secret of Dr. Vache
(Return to Natasha’s Clinic)
- [Trailblazer] I’m back. The people selling that stuff are devils.
- Natasha: Let me tell you a story, {NICKNAME}. There was once a brilliant Overworld doctor with unmatched pharmaceutical skill. The doctor was set on developing a medicine that would steel the human body against the cold. He named his research project “Blizzard Immunity.”
- Natasha: But his research met with obstruction time and time again, the biggest factor being a lack of test subjects. Tampering with the human immune system carries huge risk, and nobody in the Overworld was willing to submit themselves to such experiments. As such, the doctor got in touch with his sister in the Underworld… claiming that he wanted to support the medical effort below ground.
- Natasha: Naturally, his sister was overjoyed. After all, as far as medicine was concerned, her brother had always been the standard she’d strived to attain. She brought her brother underground and entrusted him with her clinic and laboratory… I think you know what comes next.
- [Trailblazer] It’s hard to believe he was your brother…
- Natasha: Issuing free medicine to the needy was just Vache’s way of conducting experiments on his test subjects. On some level, his reagents were able to alleviate the patients’ suffering… but nothing more. They would forget their pain for a while, and neglect their regular treatment plans… The result… patients with mild illness would attribute their recovery to the so-called wonder medicine, while patients with serious illness would quickly deteriorate… and sometimes die.
- Natasha: That’s why I want you to help me recover all the wonder medicine. This will be hard for the people that have come to rely on the wonder medicine… but it’s up to us to remove this false hope. Recover every last reagent that Vache left behind. I’ll take responsibility for the consequences.
7. Raid on the Mutual Aid Society in Rivet Town
Step Description: Natasha has decided to dismantle the mutual aid society that sells counterfeit drugs with your help. Head to Rivet Town to find where the wonder medicine comes from.
(At the entrance of Rivet Town ruins)
- Alert Vagrant: Stop right there! No one’s allowed up ahead! You’re that clinic woman’s lackey, aren’t you. Did you think the mutual aid society didn’t have its own intel? Men, look lively and send this idiot packing already!
(Defeat the guards and advance to the storage facility)
(Investigate the crates filled with numbered reagent vials)
- Society President: …Well, look who we have here. Your reputation precedes you, outsider… I didn’t take you for a bandit and a thief! I’m the president of the Complex Illness Mutual Aid Society. The medicine we hold here is lifeblood for many of our members. Are you sure you want to deprive them of that?
- Society President: The Underworld is full of sickness… What would happen if we left it all to that clinic to treat? My mutual aid society has relieved that doctor of patients at the end of the road. Shouldn’t she be thanking me?
- [Trailblazer] Natasha would never give up on a patient…
- [Trailblazer] …And she’d never work with an impostor like you!
- Society President: Men, seize the medicine thief!
(Battle against the Society President and his elite enforcers)
- Society President: Blast it… What did we ever do to you? Why are you taking away our money-maker!?
- Natasha: ”…Money-maker”? So you admit it, then? You’re not in this to save lives. You’re in it to get rich. Selling fake medicine with false kindness. You should have expected trouble to find you sooner or later.
- Natasha: I have a responsibility to protect the well-being of my patients… or to put it another way: I must eradicate any and all threats to them.
- Society President: You… You can’t save everyone! The Underworld is teeming with sickness… how could you possibly pull it off!
- Natasha: You’re right, I can’t save everyone alone… but if I must, I would sacrifice food and sleep to treat them. The debt that Vache owes to the Underworld rests with me. All I can do is help everyone to the best of my ability… to the very end.
- Society President: Ugh… So stubborn… In that sense, you’re no different from your crazy brother.
- Natasha: …I’ll take that as a compliment. Thank you, {NICKNAME}. Your actions today have saved many Underworld families. I’ll find a way to dispose of Vache’s leftover reagents. Let’s go back to town. I still owe those patients an explanation…
8. Aftermath at the Clinic & Vache’s Exile
(Return to Natasha’s Clinic in Boulder Town)
- Suspicious Patient: …What are you saying, doctor? The wonder medicine is out of supply?
- Natasha: Correct. That medicine is a wolf in sheep’s clothing — Wildfire has cut off the supply chain. Your ailments need rational, scientific treatment. Relying on pseudo-medicines will only worsen your condition…
- Suspicious Patient: Hmph, rational and scientific, huh…? The way I see it, you’re just jealous you lost your patients! If you want respect, you better find a replacement for the wonder medicine, fast! If not, I’ll tell everyone about how you set up Dr. Vache. I’m out of here!
- Natasha: …
- [Trailblazer] Are you okay?
- Natasha: Mm… I’m okay. If I were in his shoes, I may well have had the same reaction. Now I need to focus on treating the patients, and dispelling their doubts. Now’s as good a time as any to tell you Vache’s whole story.
- [Trailblazer] Was he your biological brother?
- Natasha: No… I actually grew up in the orphanage. It wasn’t until I was adopted by an Overworld family that life changed for me. My adoptive mother always saw me as her own flesh and blood, and Vache loved me like a sister… when we were young. Both of us graduated from Belobog’s medical school.
- Natasha: Vache was gentle and introverted, but when it came to work he became obsessively fixated. Ever since childhood, he wanted to develop “Blizzard Immunity” — viewing cold-intolerance as a disease to cure so humanity could leave Belobog and reclaim the Snow Plains.
- Natasha: During his stay in the Underworld, he noticed Fragmentum-corroded patients had altered thermal resistance. He began simulating Fragmentum corrosion on living patients. Those reagents were never cures; they merely masked pain while he observed changes in his lab subjects.
- Natasha: When the truth came out, Oleg and I had to decide how to handle him. The Silvermane Guards were gone, and angry mobs would have torn Vache to shreds. Oleg suggested banishment.
- Natasha: We escorted Vache to the Overworld. His final wish was to be taken to the outer Snow Plains he had always dreamed of conquering. His figure slowly disappeared into the whiteout blizzard… I’ve told myself he was dead ever since.
- Natasha: Thank you for cleaning up the mess my brother left behind, {NICKNAME}. Take this — for your trouble.
Sinossi & Eventi Chiave
Haha.
- [Trailblazer] Impari in fretta.
- Oleg: Mi lusinghi, {NICKNAME}. Ma veniamo al sodo.
- [Trailblazer] Cosa c’è, capo?
- Oleg: Hahaha, persino tu ti metti a chiamarmi così?
Grazie in anticipo!
- [Trailblazer] Come dovrei procedere?
- [Trailblazer] Nessun problema, lascia fare a me.
- [Trailblazer] Ti ho mai detto di no?
- Natasha: Sei sempre premuroso come al solito, {NICKNAME}. Buona fortuna. Saprò ringraziarti a dovere al tuo ritorno.
Paziente 1: Il Paziente Depresso
(Nelle vie di Boulder Town, vicino al Goethe Grand Hotel)
- Depressed Patient: Tu… ti ho visto alla clinica. Fai commissioni per Natasha, giusto?
- [Trailblazer] Addio copertura…
- [Trailblazer] Ehm… chi sarebbe Natasha?
E a cosa servirebbe?
- Depressed Patient: Figuriamoci, l’ho letto sui manuali!
Paziente 2: L’Anziana Paziente
(Vicino alle bancarelle di Boulder Town)
- Elderly Patient: …Come hai detto?
- Elderly Patient: Mi manca tanto.
- [Trailblazer] A me importa.
- Elderly Patient: Sei testardo, eh?
- Elderly Patient: Chi? Società di cosa? …No, mai sentita nominare…
Paziente 3: Il Paziente Sospetto
- Suspicious Patient: Una lite? No, no, è l’unico medico di buon cuore rimasto nell’Underworld.
- Suspicious Patient: La Complex Illness Mutual Aid Society vende ancora quelle scorte…
4. Resoconto a Natasha & Pareri in Clinica
- Lonely Woman: …Chi sei…? Dov’è mio marito… e dove è il mio bambino…? D—Dove sono finiti? Tu… tu lo sai dove sono, vero?
- [Trailblazer] Cerca di calmarti…
- Lonely Woman: …Non lo sai… nessuno lo sa… nessuno sa niente… Dove… dove sono andati…
- Kind Old Lady: Vieni via, creatura. È meglio lasciare in pace quella poveretta.
- Kind Old Lady: Avevano preso un malanno insolito.
Vache regalava quel veleno a tutti gratis… persino a vecchi e bambini…
- Alert Vagrant: Alt! Vietato passare!
Grazie, {NICKNAME}. Oggi hai salvato molte famiglie dell’Underworld. Provvederò io stessa a smaltire questi reagenti chimici. Torniamo in città, devo spiegazioni ai pazienti…
8. L’Epilogo alla Clinica & L’Esilio di Vache
(Ritorno alla clinica di Boulder Town)
- Suspicious Patient: …Cosa sta dicendo, dottoressa? Il farmaco miracoloso è esaurito?!
- Natasha: Esatto.
- Suspicious Patient: Cure scientifiche, eh?! Me ne vado!
- Natasha: …
- [Trailblazer] Tutto bene?
- Natasha: No… Ci laureammo entrambi all’accademia medica di Belobog.
- Natasha: Quando l’orrore venne scoperto, io e Oleg dovemmo decidere cosa fare. Oleg propose l’esilio permanente.
- Natasha: Lo scortammo nell’Overworld.
